Dr Juan Prieto, traumatologist specialist in hip surgery, hip prosthesis and managing hip pain in sports-related activities.

We deliver better outcomes because we tailor our approach to each patient’s unique needs.

The anterior hip approach

Why we deliver better outcomes in hip prosthesis?

The anterior hip approach

We are the only centre to perform all hip prostheses surgeries using the minimally invasive anterior hip approach.

THE ANTERIOR HIP APPROACH

The anterior hip approach allows surgeons to access the hip joint and replace damaged bone without disrupting muscles and tendons, making it a uniquely tissue-preserving technique.

While this method requires highly skilled hip surgeons due to its technical complexity, our team’s extensive experience and proven success enable us to offer exceptional results:

mini-invasive hip technique

Reduced postoperative pain

By minimising internal tissue damage (muscles and tendons)

mini-invasive hip technique

Decreased bleeding

Lower risk of transfusion and no need for postoperative drainage.

mini-invasive hip technique

Early recovery of walking pattern

Patients begin walking within 3 hours of surgery, as muscles and tendons remain intact.

This avoids waiting until healing for proper ambulation.

mini-invasive hip technique

Unrestricted mobility from day one

With the risk of dislocating minimised, patients can move freely, sit, sleep and put on shoes immediately after surgery.

mini-invasive hip technique

Intraoperative leg length verification:

Operating on the patient in the lying position allows us to verify that there is no alteration in the length of the legs at the time of completion of surgery.

mini-invasive hip technique

Verification of the correct position of the prosthesis:

Operating with the patient lying down ensures leg length alignment during surgery completion.

The lying position during surgery allows us to take an intraoperative radiograph (this is not routinely performed in othercenters) with the trial and definitive components, which enables us to be more accurate in the parameters, sizes and position of the components so that they completely reproduce the healthy hip, which directly influences the biomechanics and function of the hip, as well as limiting the risk of postoperative complications.

mini-invasive hip technique

Increased physical activity and mobility in the medium and long term

Reducing the risk of dislocation allows us to allow the patient to perform movements that with other techniques would have a risk of dislocation.

This means that patients who used to practice functionally demanding sports can continue to do so at a high level of demand (an important factor in recovering the quality of life that the patient desires).

mini-invasive hip technique

Aesthetic scar (bikini)

Although the scar does not determine whether the surgery is mini-invasive or not (but the degree of tissue preservation), the scar does not determine whether the surgery is mini-invasive or not.

the ANTERIOR VIA allows the realization of an aesthetic incision that is camouflaged in the folds of the groin and inside the underwear or swimsuit.

ANTERIOR HIP PROSTHESIS

In the case of osteoarthritis of the hip requiring the implantation of a hip prosthesis, hip pain and severe stiffness can substantially limit your life. Fortunately, the implantation of a hip prosthesis is a procedure that causes a degree of recovery of function and absence of pain in virtually all cases. This is not the case with other types of treatment.

The surgical technique for the implantation of a hip prosthesis has also evolved to adapt to the level of demand of today’s society. Today it allows the patient to perform more sporting activities, early recovery and above all the reduction of the risk of dislocation. All this allows us to increase the level of functional requirements of younger patients and athletes.

BILATERAL PROSTHESIS

Can both hips be operated on in the same surgery?

𝗡o it is not true that patients cannot have both hips operated on in the same surgery.

There are patients who present advanced degeneration of both hips, and their limitation is in both hips at the same time. If we were to operate on only one hip for these patients, we would not improve their quality of life since they would continue to be burdened by the other hip and would have to undergo another surgery, anesthesia, hospitalization and recovery process at another time when the other hip is operated on.

At Hip Institute we have the most experienced team of professionals in this type of interventions.

We have more than 1000 hip prosthesis surgeries and more than 10 years of experience, of which 90 patients (180 prostheses) are bilateral hip prosthesis interventions. We have become the reference center for this type of intervention and we have improved both in the operative protocol and in the recovery protocol that we can say that we do not find differences in the recovery from the first week after surgery.

It is a PRIVILEGE to be able to offer the patient such an effective solution, which allows the patient to avoid a new surgical intervention, in addition to a more surprising recovery (since there are two hips).

For more information, see the following article published in the ABC newspaper: Is it possible to operate on both hips at the same time?

ANTERIOR RESURFACING

At Hip Institute, we are leaders in offering the most innovative and effective solution for active patients who need a hip replacement: the resurfacing. We are the unique centre in Spain that offers the possibility of implanting a resurfacing hip replacement through the anterior approach, preserving the natural anatomy of the hip, reducing the risk of complications due to instability, and allowing for a faster recovery to return to activity without limitations, even for professional athletes.

Resurfacing is an advanced surgical procedure that involves replacing damaged articular cartilage without the need to cut the femoral head or modify the femoral canal. Unlike traditional replacements, this technique maintains the patient’s original bone structure, offering more natural functionality and stability.

At Hip Institute, we have taken resurfacing a step further by implementing it through the anterior approach, a minimally invasive technique that preserves tissues, reduces postoperative pain, and facilitates a faster recovery.

Hip resurfacing is ideal for patients with high levels of physical activity who seek a durable and functional solution without compromising their performance.

Resurfacing is not indicated for all patients. At Hip Institute, we conduct a personalized study to determine if this technique is the best option for you.”

ARTHROSCOPY

Why do we obtain better results in hip arthroscopies?

Our key to success is an accurate diagnosis of the cause of your hip pain, extensive experience and the application of the most innovative techniques in mini-invasive hip surgery with the goal of getting you back to your sport or daily activity in the shortest possible time.

One such minimally invasive procedure is hip arthroscopy, in which we use an arthroscope to examine the inside of the hip joint.

This mini-invasive hip surgery procedure allows us to both diagnose the cause of hip pain or other joint problems and treat some conditions. In this type of mini-invasive arthroscopic hip surgery, we make small incisions (2 to 3 incisions of less than 1 cm) to create access points for various arthroscopic needles, scalpels or other special surgical tools.

Many of the patients who come to our practice have been experiencing symptoms for a long time before they get a correct diagnosis; it is common for us to receive new patients with previous symptoms for more than 1 year, with previous visits to several physicians and/or physical therapists.

The Dr. Juan Ramón Prietosupported by a multidisciplinary team, will provide you with a correct diagnosis that will be the basis of action for the following pathologies:

INFILTRATIONS

Infiltrations are part of the NON- SURGICAL treatment of hip pathology.

We could say that it is one of the cards we can play to improve the painful symptomatology of the hip.

The objective of most infiltrations is not to cure or regenerate but to relieve (although there are nuances). In the same way that anti-inflammatory drugs seek to reduce tissue irritation in a systemic way (the whole body), infiltrations help to treat an ailment in a very focal way (where they are infiltrated).

We will talk in another post about the different types of medication that are administered in infiltrations and their characteristics: corticosteroids, hyaluronic acid, platelet factors (PRP), stem cells…

The hip infiltrations could be divided into 2 classes according to the location where they are applied:

– Intra-articular: for osteoarthritis, arthritis, labral tears, cartilage damage…

– Extra-articular: tendinitis, trochanteritis, bursitis, muscle injuries, neuralgia…

EVERY PATIENT IS DIFFERENT, so do not hesitate to consult your doctor specialized in hip pathology (Traumatologist, Rehabilitator or Sports Physician) to find out if this therapeutic option can help you in the treatment of your hip problem.

DYSPLASIA

We kept our promise!

Discover the path to a significant change in quality of life for those facing adult dysplasia. After countless hours of dedication and evaluation of numerous cases, we have distilled valuable information into an easy-to-understand book.

This book, the result of effort and experience shared with Rafael Jacome (global Fisio) and Julia Rodriguez, both experts in the treatment of dysplastic patients, focuses on providing effective and clear solutions to patients’ needs. Rafael, physiotherapist, and Julia, physician, bring their vast clinical knowledge to this work.

The illustrations, created by Ana Lozano, enrich the visual understanding of the concepts presented.

This invaluable resource is available in both digital and physical format. I highly recommend reading it to all patients diagnosed with hip dysplasia or experiencing decreased acetabular coverage.
